1. Please print clearly or type. Payment must accompany registration form and can be made by Check/Visa/MasterCard. Make checks payable to COSTDP.
2. Registration Fees are as follows:
Postmarked prior to July 1, 2008 $25.00
After July 1, 2008 thru August 31, 2008 $35.00
NO REGISTRATIONS accepted by mail after Sept 1, 2008
On Site Registration at the convention $40.00

If payment is made by Visa/MasterCard, the account number, expiration date, security code and authorized signature must be included.
You will receive a confirmation of registration for all events and classes.
4. Class fee and Teacher surface fees MUST be included with your registration payment.
Class fees are: 2 hour class $9.00 3 hour class $12.00
4 hour class $16.00 6 hour class $24.00
5. Exhibition Hall admission is FREE for registered students and $3.00 for all 3 days for non-registered guests.
6. Cancellation – Refund Policy
Refunds will be given only for cancelled classes or already filled classes. Refunds will be available in the class sales area during the convention. Notice of cancellation or class changes will be sent to the original registrant. “NO SHOWS” are NOT entitled to refunds. Transfers to another class are NOT permitted unless the original class is cancelled or full.
